Title: A Very Rare Case of Spontaneous Ovarian Hyperstimulation Syndrome Presenting During Early Labor at Term Pregnancy

She holds an Arab Board in Obstetrics and Gynecology and is an Associate Member of the Royal College of Obstetricians and Gynecologists (RCOG). She has published peer-reviewed research, presented at international conferences, and authored a book on critical care in COVID-19. She is recognized among pioneering Arab women leaders in health and medical education.
Background: Ovarian Hyperstimulation Syndrome (OHSS) is typically an iatrogenic complication of infertility treatment involving ovulation induction drugs. Spontaneous Ovarian Hyperstimulation Syndrome “Spontaneous OHSS" which occurs without ovulation induction and may or may not be associated with pregnancy is a rare condition with very few cases reported in the literature. It usually presents in the first trimester and resolves by the second trimester. The occurrence of spontaneous OHSS at a term pregnancy is extremely rare. The aim of this study is to present a rare instance of spontaneous (OHSS) occurring during early labor at term.
Case presentation: A 20-years-old primigravida at 38weeks of gestation, presented to the clinic with complaint of abdominal distention, pain, and discomfort accompanied with nausea. The pregnancy was spontaneous, with an uneventful medical and antenatal history up to three weeks prior to presentation. At that time, her ovaries appeared normal on ultrasound
On presentation Ultrasound showed a viable fetus at 36 weeks of gestation (by measurements) and bilaterally enlarged ovaries with multiple cysts. The patient was clinically stable and pelvic examination revealed signs of early labor. she delivered a healthy male infant weighing 2700 grams. via uncomplicated spontaneous vaginal delivery. Postpartum follow-up demonstrated gradual regression of ovarian size to near-normal within four weeks. To the best of our knowledge, this is only the second reported case of spontaneous OHSS. at term in the medical literature.
Conclusion: Spontaneous OHSS in term pregnancy is an exceptionally rare condition. This case underscores the importance of considering spontaneous OHSS in the differential diagnosis of ovarian enlargement in late pregnancy, even in the absence of fertility treatment. To the best of our knowledge, this is only the second such case reported in the literature.
