A Case of Secondary Infertility with Ovarian Endometriotic Cysts: Successful Pregnancy and Management of Suspected Partial Hydatidiform Mole


Biography

Shaomin Yu is a dedicated Obstetrician-Gynecologist at Yubei District People’s Hospital in Chongqing, specializing in reproductive endocrine disorders. She has extensive experience in the diagnosis and management of a wide range of conditions, including infertility, polycystic ovary syndrome (PCOS), menstrual irregularities, diminished ovarian reserve, and perimenopausal syndrome.

Abstract

Case description: A 33-year-old female patient, diagnosed with secondary infertility and ovarian endometriotic cysts, achieved a successful pregnancy following ovulation induction therapy using human menopausal gonadotropin (HMG). At five weeks of gestation, ultrasound examination showed multiple small anechoic areas near the gestational sac, suspected to be partial hydatidiform mole (PHM). Due to the pregnancy's preciousness, close observation was recommended.

At eighteen weeks of gestation, amniocentesis karyotype analysis revealed 46,X?, indicating a normal chromosome. This genetic confirmation played a crucial role in reassuring the patient and guiding decisions.

Outcome: The patient delivered a 2880g healthy baby girl via cesarean section at 37+6 weeks. Both the mother and infant were discharged in good condition.

Conclusion: This case emphasizes that for complex early pregnancy ultrasound findings, a cautious, evidence-based approach with genetic testing is crucial. It helps avoid misdiagnosis and preserves valuable pregnancies. This case provides valuable experience for the management of endometriosis combined with rare ultrasound abnormalities during pregnancy.
