Incidental
ovarian cysts are frequently discovered during gynecological and
non-gynecological surgeries. The decision to remove or preserve these cysts can
significantly influence ovarian reserve and future fertility outcomes. This
presentation highlights clinical case scenarios where unnecessary cystectomy or
oophorectomy resulted in reduced ovarian function. Current evidence and
international guidelines will be reviewed to emphasize when ovarian cyst
removal is indicated and when conservative management is preferable. The
discussion will also address the impact of surgical techniques—including
hemostasis methods and the use of energy—on ovarian tissue preservation. The
goal is to provide practical recommendations for optimizing surgical
decision-making, avoiding unnecessary ovarian damage, and protecting fertility
potential in reproductive-age women.
